There will come a time when you, your parents and your medical team feel that you are ready to move from care by a paediatric (children’s) team to an adult team. You may have lots of questions about the move. The aim of this Crohn’s Colitis NZ website is to help you through this transition.

How will I feel?

Moving to adult services can make you feel more in control because you get to go to appointments alone and make your own decisions.

This can be really good for you but sometimes you can feel alone or uncertain and you may want your parents’ or carer’s support.

Just remember that it’s okay to get support when you want it.

It’s totally normal to take some time to get used to the change.

Becoming confident in adult services is a process and no one expects you to become an expert overnight!
